What is the main twist in Andhadhun?

Akash (Ayushmann Khurrana) is not actually blind; he has been faking his blindness for musical inspiration and a competitive edge, a fact he conceals even from his girlfriend Sophie. This is revealed early when he witnesses the murder of Pramod Sinha (Anil Dhawan) by his wife Simi (Tabu) and her lover Inspector Manohar (Manav Vij), but he pretends not to see.

What happens in Andhadhun?

Akash Deshpande is a visually impaired pianist living in Pune. From the start, he is shown navigating his apartment with apparent blindness, using a cane and asking neighbors for help. He performs at a small restaurant where he meets Sophie, a waitress, and they begin a romantic relationship. Akash uses his pretended blindness as a selling point for his music career, garnering sympathy and attention.

One day, the retired actor Pramod Sinha hires Akash to play piano at his home for their wedding anniversary. Akash visits the Sinha residence and meets Pramod and his younger wife, Simi. During his first session, Simi tests his blindness by suddenly appearing in front of him and waving her hand; Akash acts as though he does not notice, passing the test. Simi invites him back for a private concert.

On his second visit, Akash plays the piano while Simi and her lover, Inspector Manohar, kill Pramod in the next room. Akash hears the struggle and the gunshot but continues playing. When Simi and Manohar discover he was present, Simi tests him again by placing a pair of gloves in front of him; he does not react. She then pretends to put something in his drink while he is at the piano, but Akash, seeing through his peripheral vision, swaps the glasses. Simi later realizes he is not blind when she returns to find a contact lens on the floor, leading her to suspect he saw everything.

Simi and Manohar attempt to kill Akash by pushing him off a balcony, but he survives. He goes to the police station to report the murder, only to find that Manohar is the investigating officer. Manohar forces Akash to sign a false statement claiming he saw nothing. Later, Simi and Manohar try to kill him again by placing him in a car and sending it into a river, but Akash escapes.

After this, Akash is kidnapped by a gang led by a man named Swami, who turns out to be a doctor involved in illegal organ harvesting. Akash is taken to a warehouse along with a woman named Murli (or Mita) who also has been captured. Akash manages to trick the gang into thinking he is truly blind and then escapes with the woman. In the process, Swami is killed.

Akash then tracks down Simi, who has been hiding. He confronts her at her home, pretending to still be blind. Simi admits to the murder and tries to kill Akash by injecting a chemical into his eye. A struggle ensues, and in the chaos, Simi is killed by the falling of a window? Actually, she dies when a shard of glass pierces her neck after a fight. Akash is injured and his eyes are temporarily damaged.

He is found by doctors and treated. Later, we see Akash in London, where he meets Sophie again. He tells her a story about what happened: he says Simi died in a car accident and that he went to Prague. The story is full of gaps and may be a lie. As he finishes, Sophie notices that Akash uses his cane to strike a tin can placed on the ground with great precision, implying he may still be faking his blindness or that he has regained his sight. Sophie smiles and walks away. The camera lingers on Akash, who then strikes another can with his cane, the final shot leaving his condition ambiguous.
