What is the main twist in Inception?

Cobb (Leonardo DiCaprio) is haunted by a projection of his deceased wife Mal (Marion Cotillard), which is actually a manifestation of his own guilt. The central twist is that Cobb performed inception on Mal years earlier, planting the idea that her world was a dream, which led to her suicide in reality. The spinning top totem at the end may still be spinning, leaving ambiguous whether Cobb is in a dream or reality.

What happens in Inception?

Dom Cobb is an extractor, a skilled thief who steals secrets from people’s subconscious by entering their dreams. He is unable to return to his children in the United States because he is accused of murdering his wife Mal. A powerful businessman named Saito (Ken Watanabe) offers Cobb a chance at clearing his criminal record if he can perform inception—planting an idea in someone’s mind rather than stealing one. The target is Robert Fischer (Cillian Murphy), the heir to a vast energy empire. Cobb assembles a team: Arthur (Joseph Gordon-Levitt) as the point man, Eames (Tom Hardy) as the forger who can mimic people in dreams, and Yusuf (Dileep Rao) as the chemist who creates the sedative. They also bring in Ariadne (Elliot Page) as the architect who designs the dream levels.

The team plans to descend through three layers of dreams within a shared dream, each level with its own time dilation. They kidnap Fischer in first class on a flight from Sydney to Los Angeles. In the first dream level, a rainy city, they are attacked by Fischer’s trained subconscious projections. Saito is wounded, which will cause him to fall into a limbo-like state if he dies in the dream. They drug Fischer and push deeper into a second dream level: a hotel. There, Eames impersonates Fischer’s godfather Peter Browning to create distrust. Cobb convinces Fischer that his subconscious is being attacked and that they are his protectors. He suggests they enter Browning’s mind to uncover the truth, which actually takes them to the third dream level: a snowy mountain fortress.

In the third level, they need to reach the stronghold where Fischer’s relationship with his father is stored. Meanwhile, Mal’s projection appears, sabotaging Cobb. She shoots Fischer, causing him to sink into limbo. Ariadne and Cobb follow Fischer into limbo to rescue him. In limbo, Cobb confronts Mal, admitting that he performed inception on her by hiding the top totem in her safe and convincing her that her reality was a dream. Mal had believed she was still dreaming even after waking, and she killed herself to “wake up,” forcing Cobb to flee the country. Cobb finally lets go of his guilt and accepts that Mal is dead. Ariadne kills Mal’s projection, allowing Cobb and Fischer to return to the third level. Fischer sees his dying father in the snow fortress, who tells him to “create something for yourself” and then reveals a childhood pinwheel, confirming the planted idea to dissolve the company.

The team kicks up through the dream levels: Yusuf drives the van off a bridge in level one to create a kick; Arthur uses elevator explosives in the hotel level; Eames blows up the fortress in the snow level. Cobb stays behind in limbo to rescue Saito, who has aged into an old man. Cobb finds Saito in a crumbling version of Cobb’s beach house. Cobb reminds Saito that they came to “take a leap of faith.” The kicks sync and everyone wakes up on the plane. Saito makes a phone call, and Cobb passes through customs at LAX, finally free. He returns to his father’s house and sees his children’s faces for the first time. Cobb spins his totem top but does not wait to see if it falls, instead hugging his children. The top wobbles on the table as the screen cuts to black.
